    my try to reinstall MOD again .. i think my currant MOD is break somewhere .. and can you tel me your test MOD enable PHP opcache ? your WP install in 22 Centmin Menu ?

    I mean i was updrage and downgrade PHP version and etc (many times ).and its may case the issue didnt get logs like yours .. so let me try reinstall MOD .. Frash install and check the problem still present ..
