Centmin Mod Sites Database and Form Added

Added a new Sites Database page to the forums which includes a submissions form to submit your sites' details. The database listing is also displayed on the same page and is automatically updated as new submissions are made - there maybe a delay before the listing shows up and a page fresh is needed.

Sites Database page is viewable to all registered forum members only. So you will need to register on the forums to access the page
